REQUIRED MetLife Life & AD&D Insurance Change

ATTENTION: You must take action by November 30, 2016. Your beneficiaries will not transfer to our new Life Insurance Provider. It’s easy to update!

  • Online through MyBenefits, Using PEBB Benefits – State of Washington as your employer or
  • By submitting the MetLife Enrollment Change form for Enrollment Period from November 1, 2016 – November 30, 2016
  • Forms must be received by MetLife no later than Wednesday, November 30, 2016
  • If you are not making changes to your life insurance, logon to the MetLife MyBenefits portal (using PEBB Benefits – State of Washington as your employer) and name your beneficiary or complete the MetLife Beneficiary Designation form.
  • Direct questions about life insurance to MetLife customer service at 1-866-548-7139.
  • Review the HRS Open Enrollment website for additional details.
