PEBB eligible members: It’s Not Too Late!

You can still qualify to receive an extra 100 bonus points towards your Wellness Incentive by completing your SmartHealth Wellness Assessment before March 31, 2015.

You will earn 800 points by simply taking your assessment; if completed before the end of March, you will receive the Early Bird Bonus, of an additional 100 points! The Wellness Assessment is required in order to be eligible for the $125.00 incentive to be earned in 2016. You must earn 2,000 points total before June 30, 2015.
