Instructions to complete your COVID‑19 vaccination declaration

Here’s how to find where in Workday you can complete your COVID-19 vaccination declaration.

  • In Workday, select “Requests” from the welcome page and then “Create Request
  • In the “Request Type” window, type “COVID” and press enter
  • Select “COVID-19 Employee Vaccination Declaration” and click “OK”
  • Read the declaration statement and select the statement appropriate to you. Then click “Submit.”

Human Resource Services has produced a handy, detailed guide complete with images of the key steps that can be accessed from its website.

All employees must declare themselves fully vaccinated in order to forgo face masks and physical distancing while at a WSU location. Someone is considered fully vaccinated two weeks after receiving their final dose of a vaccine sequence.
