PEBB Eligible Employees: Your 2014 Statement of Insurance Is Available Online!

The Health Care Authority (HCA) has granted access to PEBB eligible employees to print their own Statement of Insurance online.

This statement includes medical and dental enrollment information for the subscriber and subscriber’s family members. Enrollment information on life insurance and long term disability benefits can be accessed through ZZUSIS, under Employee Benefits within the Payroll section.

The Statement of Insurance can be accessed by going to www.hca.wa.gov/pebb and clicking “My Account” on the left hand side. If they have not already done so, employees will be asked to register and create a username and password. The employee can then print or save the document at any time. For employees who do not have computer access, Benefit Services would be happy to access this statement for the employee.
