The new testing changes simplify and streamline previous procedures.
- Online Exam Request form
Eligible students with exams to be proctored at the Access Center will submit an Online Exam Request Form, for each exam, no less than seven days before the in-class exam. Once submitted, the form generates email notifications to the instructor and the Access Center of student intent to complete the exam at the Access Center. If instructor will be proctoring the student’s exam or within their department then no Online Exam Request Form is needed.Please note: Instructors proctoring their own exam(s) for eligible students must provide all testing accommodations listed on the Accommodation Form. Contact the Access Center for additional information or assistance, if needed.
- Alternate Exam Date/Time Approval Form
All students are expected to begin their exams at the same time as the in-class exam. However, there will be exceptions.In these situations the student will meet with instructor to obtain signed approval on the Alternate Exam Date/Time Approval Form.A receipt will be emailed to the instructor once the student has returned the form to the Access Center. - Faculty Proctoring Instruction Form
A new Faculty Proctoring Instruction Form is available at https://accesscenter.wsu.edu on the right-hand side of the page under “Quick Links”.A Proctoring Instruction Form needs to accompany all exams sent or delivered to the Access Center. - Completed Exam Return Option
The option to have completed exams scanned then emailed to instructors has been reinstated. This completed exam return option has no page limit and does not exclude final exams.If you would like the Proctoring Office to password protect the scanned exam document before it is emailed please contact Teresa Davi, Alternative Testing Services Coordinator at access.testing@wsu.edu.
