Make sure you completed the vaccination verification process

Monday, Oct. 4 is the deadline for employees to complete the COVID-19 vaccination verification process via Workday.

In order to be fully vaccinated by Oct. 18 – the deadline for state workers – WSU employees need to have their second dose of either the Pfizer or Moderna vaccine or the one-dose Janssen/Johnson & Johnson by Oct. 4.

Employees who are vaccinated must complete the Workday process and show proof of vaccination to their supervisor. There are several forms of acceptable documentation:

  • CDC COVID-19 Vaccination Record Card or photo of the card;
  • Documentation of vaccination from a health care provider or electronic health record;
  • State immunization information system record; or
  • For an individual who was vaccinated outside of the United States, a reasonable equivalent of any of the above.

Employees may request an exemption in Workday for those with sincerely held religious beliefs or with documented medical reasons.

After the Oct. 4 deadline passes, the university will begin initiating the appropriate separation process for employees who have not taken action.

Whether verifying vaccination status or requesting an exemption, the process in Workday is straightforward:

  • Log into Workday
  • Within the applications window, click Requests
  • Select Create Request
  • In the Request Type field, type COVID-19 Vaccination Verification and click okay
  • Carefully read the statements and select the statement that reflects your situation
  • Submit the request

In addition to requesting the exemption request in Workday, the employee must submit supporting information.  Medical exemption request forms are completed and emailed to Disability Services at hrs.disabilityservices@wsu.edu or faxed to 509-335-1259. Religious exemption request forms are emailed to hrs.exemptions@wsu.edu or faxed to 509-335-1259.

Students provide proof of vaccination or request a religious or medical exemption through the process outlined on Cougar Health Services’ website.
