LTD Open Enrollment: How do I enroll or change my optional LTD Waiting Period during Open Enrollment?

All LTD Open Enrollment changes are done by completing and submitting the Open Enrollment Change Form. Hard copies of this form can be found at HRS Benefits, French Administration 139 in Pullman.

Forms must returned no later than Friday, March 29, 2019 and can be submitted multiple ways:

USPS Mail: HRS Benefits, PO Box 641014, Pullman, WA 99164-1014

Campus Mail: 1014

Fax: 509-335-1259

Or hand deliver to your campus’ HRS office.

If you attempt to enroll in LTD coverage or shorten a waiting period in the future, you will be required to fill out an evidence of insurability form to be reviewed for possible approval by the insurance company.
