On-line Request for Shipment of Merchandise Tips

A new electronic Request for Shipment of Merchandise form is now available on the WSU E-Forms System. Here are a few important tips to follow when using the form.A call must still be made to Central Receiving for a pick-up when you have a package to ship off campus.

  1. Please clearly write the request number for your shipment next to the address label.
  2. If FedEx picks up a package directly from your department, please input the carrier name and tracking number in the appropriate section located in the upper right side of the form.
  3. All completed Request for Shipment of Merchandise must be sent to Central Receiving via work flow.
