PEBB Eligible Employees: What is a primary care provider and how do I establish a relationship so I can be eligible for the SmartHealth Wellness Incentive?

The PEBB Program is offering a new wellness incentive program called SmartHealth. To earn the wellness incentive for the 2015 plan year, subscribers need to do three things:

What Is Primary Care Provider and How Do You Establish A Relationship?

A primary care provider (PCP) is a physician which takes care of a patient’s basic needs across a wide continuum of different problems. He or she is the first point of contact for a person with a medical or health concern and gets to know the patient personally over time. The primary care provider cares about the patient’s whole health package including aspects of preventative care such as immunizations, mammograms and colonoscopies. This provider makes sure the patient receives the proper care and coordinates with specialists as needed.  A PCP can provide regular advice on healthy lifestyle habits you can start now for a healthier future and improved quality of life.

To find a PCP, if you do not already have one, call your medical plan’s customer service or select a provider through your plan’s online directory.
