Simplified life insurance application during Open Enrollment

MetLife, the state’s life insurance vendor, is offering a simplified life insurance application process for employees during the Nov. 1–30 Open Enrollment period for 2024.

Instead of filling out a full Statement of Health document, which normally is required to apply for additional life insurance, employees can request coverage by answering five health questions. However, if you wish to request coverage on your spouse or state-registered domestic partner, they will be required to fill out the Statement of Health document and are not eligible for the simplified 5-question application process.

To apply, visit MetLife for PEBB or call 866-548-7139. More information on life insurance is available at PEBB MetLife Employee Benefits.
