Washington State University has launched a new attestation website for students, staff and faculty as well as visitors to any of its campuses or worksites. Though WSU introduced its self-attestation program to employees in late June, the new website provides one convenient location for accessing the required attestation forms, regardless of university affiliation.

Based on guidance from the Washington State Department of Health, the Office of Financial Management, and Gov. Jay Inslee’s Safe Start Washington Employer Requirements, WSU is requiring anyone who plans on being on university’s property to complete an attestation form. That includes all campuses, extension locations and satellite offices.

This form must be completed daily prior to visiting a physical WSU location.

“Self-attesting daily is one of many important actions that each of us can take to help contain the spread of COVID-19,” Jill Creighton, associate vice president & dean of students, said.

Self-attestation is just one part of this effort. Anyone visiting a physical WSU site must also wear a mask, physically distance at least six feet from others and wash their hands frequently. As part of attesting to their health, members of the WSU community and visitors should also take their temperatures before attesting to ensure they don’t have a fever.

Anyone experiencing symptoms of COVID-19 as defined by the CDC is asked to stay home and call their healthcare provider.

A mobile MyWSU app is also available for WSU students, faculty and staff to attest while on the go. It can be downloaded on the App Store or Google Play. Those who complete a daily attestation will receive a randomized number and letter on a confirmation screen, which indicates they completed the health screen on a specific day.

Anyone with technical questions can contact the Crimson Service Desk via email at crimsonservicedesk@wsu.edu or by calling 509-335-4357.

Students with questions on attestation are asked to contact the Office of the Dean of Students. Faculty, staff and employees can get questions answered by reaching out to Human Resource Services. Vendors should contact purchasing@wsu.edu for questions about attestation. All other visitors should contact their WSU liaison directly.
