PEBB Eligible Employees: Your 2014 Statement of Insurance Is Available Online!

The Health Care Authority (HCA) has granted access to PEBB eligible employees to print their own Statement of Insurance online.

This statement includes medical and dental enrollment information for the subscriber and subscriber’s family members. Enrollment information on life insurance and long term disability benefits can be accessed through ZZUSIS, under Employee Benefits within the Payroll section.

The Statement of Insurance can be accessed by going to www.hca.wa.gov/pebb and clicking “My Account” on the left hand side. If they have not already done so, employees will be asked to register and create a username and password. The employee can then print or save the document at any time. For employees who do not have computer access, Benefit Services would be happy to access this statement for the employee. Please contact Benefit Services at 509-335-4521 or HCA at 1-800-200-1004 with questions.

The Notices and Announcements section is provided as a service to the WSU community for sharing events such as lectures, trainings, and other highly transactional types of information related to the university experience. Information provided and opinions expressed may not reflect the understanding or opinion of WSU. Accuracy of the information presented is the responsibility of those who submitted it. The self-uploaded posts are reviewed for compliance with state statutes and ethics guidelines but are not edited for spelling, grammar, or clarity.
