PEBB Eligible Employees: SmartHealth Wellness Incentive – Eligibility Requirements

You are aware of the SmartHealth Wellness Incentive that will allow employees to reduce their PEBB medical plan deductible by $125, or have an additional $125 added to their DCHP Health Savings Accounts, but just what are the requirements?

  1. Choose a primary care provider (PCP):  A PCP is a health care practitioner who people see for their common medical problems, or for annual check-ups.  This person is usually a doctor, physician assistant or a nurse practitioner.  A PCP can provide regular advice on healthy lifestyle habits you can start now for a healthier future and improved quality of life.
  2. Complete a PEBB Health Assessment:  All the PEBB plans have a health assessment that you can take on their websites.  It will take approximately 20-30 minutes to complete.  The assessments will help draw attention to potential health risks you may have, and how to reduce them.  Visit the PEBB – Take Your Health Assessment website.
  3. Start a Wellness Activity:  PEBB has identified the following as approved activities:  enrollment in the diabetes prevention program, diabetes control program, tobacco cessation program, or weight-management program; or do 90 minutes of moderate to vigorous physical activity per week for 10 consecutive weeks; or eat six or more servings of fruits and vegetables per day for 10 consecutive weeks; or work with your primary care provider to get all recommended preventative care.

Between April 1 and June 30, your must attest to having completed all three steps by logging in to your HCA My Account  in order to receive your wellness incentive for the 2015 plan year.
