PEBB ELIGIBLE EMPLOYEES: It’s April 1, Now What Do You Do?

Starting today and going through May 15, employees are required to log-on to their Health Care Authority’s “My Account” to answer questions regarding Premium Surcharges and a Wellness Incentive.  Click here for instructions on how to register for “My Account” if you have not done so already.

Once logged in to the HCA “My Account,” employees will be prompted to answer the following:

Premium Surcharges

  • If you have a spouse or domestic partner on your coverage, does the spouse or domestic partner coverage surcharge apply?
    • This portion will ask you to complete the Spousal Plan Supplemental Questionnaire and, if necessary, the Spousal Plan Calculator.
  • If you are enrolled in medical coverage, have you or your covered family members (if applicable) used tobacco products in the last two months?


SmartHealth Wellness Incentive

  • Can you confirm whether you have:
    1. Chosen a primary care provider
    2. Completed your PEBB medical plan’s health assessment
    3. Started a wellness program activity approved by PEBB

 

Depending on your responses, you will be informed if you will be assessed a premium charge(s), or if you are eligible for the wellness incentive.
