Human Research Protection Program – Updated applications available

Changes to the Federal Regulations for the Protection of Human Subjects in research (also known as the Revised Common Rule) are scheduled to take effect on January 21.  In order to comply with these revised Federal Regulations, the WSU Institutional Review Board Human Subjects Protection Program (IRB – HRPP) is updating some of its Forms and Templates.  All protocols received or scheduled for review after 5 pm on January 18, 2019 will need to use the following revised forms as applicable:

  1. Exempt Application
  2. Non-Exempt Application
  3. Consent Template
  4. Assent Template (Ages 7-10)
  5. Assent Template (Ages 11-14)
  6. Assent Template (Ages 15-17)
  7. Parent Permission Form Template
