Enroll children for insurance within 60 days of birth

Newborn or newly adopted children must be enrolled on WSU employees’ insurance within 60 days of birth or the date on which you assume legal responsibility.
To add dependents to health coverage, you must submit a medical and dental enrollment/change form to Human Resource Services (HRS). The form is available ONLINE

www.hrs.wsu.edu/benefitforms and at campus HRS offices.
Dependents will need Social Security numbers. If the 60-day enrollment window is about to expire, and you have not received your child’s number, contact HRS Benefits as soon as possible.
Late enrollment normally is not allowed and could result in dependents going without coverage until the next open enrollment opportunity.
