As of Jan. 1, a new online form makes it easier to report workplace injury or illness. Once completed, the form is electronically routed to Human Resource Services, Environmental Health & Safety, and the employee’s supervisor.
A revised section in the Safety Policies and Procedures Manual (SPPM Section 25.20) describes how to report incidents using the new system.

Safety committees still will receive a copy of incident reports for review. The person preparing the report will print off a hard copy and route ti to the safety committee chair.
